Job summary

CVS Health is seeking a Case Manager Registered Nurse - Field to conduct home visits and coordinate care for members in Bergen County, NJ, with up to 75% travel. The role emphasizes assessment, care planning, and documentation within an MLTSS/D-SNP framework.

Required are 3+ years of clinical practice, NJ RN license, NJ Choice certification, and strong communication skills. This is a full-time field/remote position with a 8am-5pm EST schedule.

Qualifications

Must reside in Bergen County, NJ and hold an active RN license.
Experience in clinical settings or home health is required.
Successful completion of NJ Choice certification is required.
Must be willing to travel up to 75% within Bergen & surrounding counties.

Responsibilities

Conduct face-to-face visits in members' homes using comprehensive assessment tools for MLTSS/D-SNP and non-MLTSS.
Develop care plans and authorize services in a cost-effective manner with the care team.
Document accurately in the member's electronic health record.
Coordinate with members, PCPs, and care team, advocating for safe discharges.
