Job summary

CVS Health is hiring a Field/Remote Case Manager RN to conduct home visits across Union County and surrounding counties in New Jersey. The role involves assessing member needs, coordinating with care teams, and promoting cost-effective, quality care with strong documentation. Expect up to 75% field travel and a Monday–Friday schedule.

Required are an active NJ RN license, 3+ years of clinical practice, and reliable transportation. NJ Choice certification completion is a contingency of employment.

Qualifications

Active and Unrestricted RN license in NJ.
3+ years clinical practice experience in hospital, home health, or ambulatory care.
Must reside in Union County, NJ.
Willing and able to travel 50-75% in Hudson and surrounding counties, NJ.

Responsibilities

Conduct face-to-face visits in members' homes using assessment tools for MLTSS and D-SNP/FIDE or non-MLTSS members.
Develop, implement, and promote health service strategies and programs.
Coordinate care with PCPs and care team; mentor new hires as proficiency grows.
