CVS Health is seeking a Field/Remote Case Manager in New Jersey to assess, plan, and coordinate care for members enrolled in MLTSS/Dual SNP programs. You will perform home visits, collaborate with care teams, and ensure cost-effective service utilization in a fast-paced environment.

Ideal candidates have 3+ years clinical RN experience, NJ license, and willingness to travel 75% within Hudson and surrounding counties; strong writing and communication skills are essential.

Qualifications

• Must reside in Hudson or surrounding counties in New Jersey; 3+ years clinical practice.
• Active and Unrestricted RN license in NJ.
• Reliable transportation; travel up to 75% in Hudson and surrounding counties.
• NJ Choice certification required; exam score 80%+ and training modules completed.
• Education: Associate’s degree or Nursing diploma.

Responsibilities

• Conduct face-to-face visits in members' homes with comprehensive assessments.
• Develop care plans and authorize services within MLTSS/FIDE programs.
• Coordinate care with PCP and team to ensure safe discharge and appropriate services.
• Document accurately in the member’s electronic health record.
• Mentor new hires once proficient.
