- At least 2 years of experience working in a licensed home care organization.
- At least 2 years in a care-coordination role or similar.
- Ability to handle fast-paced, high-volume workload.
Responsibilities

- Review and update care plans based on feedback.
- Support caregivers with ongoing coaching.
