Vynca Inc. is seeking a Lead Care Manager to coordinate care for clients with complex needs. The role partners with healthcare providers, social services, and families to ensure timely, person-centered care. You will build care plans, advocate for clients, and document outcomes across telehealth and in-person settings.

This position emphasizes collaboration with the ECM team, flexible scheduling within Pacific Time, and a focus on access, quality, and cost-effective services for home-based care.

Qualifications

• 2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations
• Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time with flexibility for evenings/weekends
• Working knowledge of government and community resources related to social determinants of health
• General computer skills and working knowledge of Google Workspace, MS Office and the internet
• Bilingual English/Spanish preferred

Responsibilities

• Assess member needs across physical health, mental health, SUD, oral health, palliative, memory, trauma-informed care, social supports, housing, and linkage to services
• Oversee development of client care plans and goal setting
• Provide services where the member resides or can access, including telehealth or field-based
• Connect clients to social services and supports as needed
• Advocate with healthcare professionals using evidence-based practices
• Conduct outreach to facilitate linkage to the ECM program and log activity in CRM
• Complete documentation and outcome measures within care plan timeframes
• Maintain up-to-date health records in the EMR and other systems
• Attend training as assigned
