Job summary

Vynca Inc. is seeking a Lead Care Manager to coordinate care for individuals with complex needs. The role partners with providers and social services to ensure timely, person-centered care and optimal use of benefits.

Expect telehealth, office, or field-based services to meet member needs while promoting wellness and independence. The LCM will develop care plans, advocate for patients, perform outreach, and maintain documentation and EMR accuracy.

Qualifications

• 2+ years experience as a care manager or equivalent role supporting vulnerable populations.
• Willing to work Monday–Friday 8:30am–5:00pm PT with potential evenings/weekends.
• Knowledge of social determinants of health and community resources.
• General computer skills and familiarity with Google Workspace and MS Office.
• Bilingual English/Spanish preferred.

Responsibilities

• Assess member needs across physical health, mental health, SUD, oral health, palliative care, memory care, and social supports.
• Oversee client care plans and goal setting.
• Provide services where the member resides (office, telehealth, or field-based).
• Connect clients to social services and advocate with health professionals.
• Conduct outreach, log activity in CRM, and complete documentation within care plans.
• Maintain up-to-date EMR and attend required trainings.
