Vynca Inc. is seeking a Lead Care Manager to serve as the primary coordinator for clients, collaborating with healthcare providers and social service agencies to ensure seamless, person-centered care.

The role encompasses case management, benefits coordination, and education to promote wellness and independence. The LCM will work with caregivers and care teams to maximize member benefits, provide timely services, and navigate care plans across home, telehealth, and community settings.

Qualifications

• This role requires 2+ years of experience in care management or related field.
• Experience coordinating care across multiple providers and settings.
• Comfort with telehealth, field visits, and office-based services.

Responsibilities

• Assess member needs across physical health, mental health, SUD, oral health, and housing.
• Oversee development of client care plans and goal setting.
• Provide services where the member resides, including telehealth or field-based care.
• Connect clients to social services and supports as needed.
• Advocate for clients with healthcare professionals using evidence-based approaches.
• Conduct outreach and log activity in CRM system.
• Complete all documentation within established timeframes.
• Maintain up-to-date health records in EMR and other systems.
• Attend required training.
