Job summary

Vynca Inc. is seeking a Lead Care Manager to coordinate high-need clients with a focus on home-based, person-centered care.

You’ll partner with healthcare providers, social services, and families to develop care plans, link to resources, and ensure timely access to benefits. The role covers in-office, telehealth, and field-based visits, with scheduled Pacific Time hours Mon-Fri 8:30am-5:00pm, and potential evenings or weekends.

Qualifications

• 2+ years experience as a care manager or similar role.
• Willing to work Pacific Time 8:30am-5:00pm with flexibility for evenings/weekends.
• General computer skills and knowledge of Google Workspace and MS Office.
• Bilingual English/Spanish preferred.

Responsibilities

• Assess member needs across physical health, mental health, SUD, oral health, palliative and memory care, housing and social supports.
• Oversees the development of the client care plans and goal settings.
• Offer services where the member resides, including office-based, telehealth, or field-based.
• Connect clients to social services and supports; advocate with health care professionals.
• Maintain up-to-date patient health records in the EMR and other systems.
• Complete documentation and log activity in CRM; attend training as assigned.
