Qualifications

• 2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations.
• Working knowledge of Google Workspace, MS Office and the internet.
• Preferred bilingual English/Spanish.

Responsibilities

• Assess member needs in physical health, mental health, SUD, housing, and social supports.
• Oversee development of client care plans and goal settings.
• Connect members to community-based services and supports.
• Advocate on behalf of clients with health care professionals.
• Complete documentation and log activity in CRM; maintain EMR records.
