Zusammenfassung

Vynca Inc. is seeking a Lead Care Manager to coordinate patient-centered care for individuals with complex needs. You will partner with clinicians, social services, and caregivers to develop and implement care plans, manage benefits, and ensure timely access to services in home-based or telehealth settings.

You'll work closely with the ECM team to advocate for clients, document outcomes, and maintain accurate EMR records while supporting families and care teams across Pacific Time hours.

Qualifikationen

• 2+ years of experience in care management or related field
• Experience supporting vulnerable populations
• Proficient with Google Workspace and MS Office

Aufgaben

• Assess member needs across physical/mental health and social determinants
• Develop client care plans and goals with the care team
• Deliver services in office, telehealth or field settings
• Connect clients to social services and supports
• Advocate with health care professionals using evidence-based approaches
• Conduct outreach to facilitate ECM program linkage and log activity in CRM
• Complete documentation and outcome measures on plan timelines
• Maintain up-to-date patient records in the EMR system
• Attend required training
