Job summary

Humana Inc is recruiting a Telephonic Registered Nurse Care Manager to support CenterWell Home Health patients remotely with a 40-hour week over four 10-hour shifts, plus occasional travel for training. You will guide care transitions, assess post-acute needs, and collaborate with clinical teams to ensure timely referrals and admissions.

The role requires an Associates degree, a compact state RN license, and at least 3 years of post-acute nursing experience; BSN preferred, with knowledge of home

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license required; additional licenses may be obtained as needed.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Strong communication with empathy over the phone.

Responsibilities

• Be a CenterWell Home Health representative for discharged patients; navigate post-acute options.
• Provide patient options and education on post-acute care, Medicare coverage, and related questions.
• Make clinical level-of-care determinations based on medical data and arrange services accordingly.
• Support admissions and ensure timely referrals and follow-up to optimize admission conversions.
• Adhere to HIPAA privacy and Company policies; participate in training.
• Educate patients and families about Home Health, Hospice, and Palliative Care services.
• Participate in special projects and other assigned duties.
