Job summary

Humana Care Support is seeking a Telephonic Registered Nurse to act as a Care Manager. You will assess, plan, coordinate, and monitor care for Medicare members remotely, helping them stay independent at home.

The role requires an active RN license in the proper state, at least 3 years of acute care and chronic condition management experience, and strong communication skills for telephonic engagement.

Qualifications

• Active RN license in state of residence with multistate licensure.
• 3+ years of acute care experience including case management and discharge planning.
• Experience managing chronic conditions with assessment, care coordination, and individualized care planning.
• Strong clinical assessment, critical thinking, and decision-making skills.
• Proficiency with Microsoft Office and the ability to navigate multiple systems.
• Ability to manage multiple priorities in a telephonic care setting.
• Excellent verbal and written communication for telephonic/virtual interactions.

Responsibilities

• Provide telephonic care coordination for Medicare members.
• Assess members' health, environment, behavioral and psychosocial needs.
• Develop, implement and monitor individualized care plans.
• Connect members with clinical, community and social resources.
• Coordinate care with interdisciplinary teams to improve outcomes.
• Utilize clinical judgement to prevent avoidable hospitalizations.
