Job summary

Humana Inc. is seeking a Telephonic Registered Nurse Care Manager to support patients transitioning into and out of services from a remote work setting.

You will guide post-acute options, ensure continuity of care, and act as a liaison with our clinical team to optimize referrals and admissions. Responsibilities include advising on home health, hospice and palliative care, performing careful care determinations, and educating patients by phone with empathy.

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license required.
• At least 3 years post-acute nursing experience.
• Home health or hospice experience preferred.
• Knowledge of home health, hospice and palliative care.

Responsibilities

• Be a CenterWell Home Health representative in supporting patients who have been discharged from service or for those who may need post-acute services.
• Navigate healthcare options and provide patient education including post-acute care options and Medicare coverage.
• Make clinical level of care determinations based on discussion, records and data.
• Maintain awareness of department performance objectives and ensure patient satisfaction goals are met.
• Assist in admissions by acting as an ambassador for patients meeting admissions requirements.
• Place the right patient to the right care setting at the right time.
• Adhere to mandatory training including HIPAA privacy, ethics and company policies.
• Review and adhere to all company policies and procedures.
• Provide education regarding Home Health, Hospice, and Palliative Care Services.
• Assist with clinical eligibility review for alternate services.
• Participate in special projects and other responsibilities.
