Job summary

CenterWell Home Health, a Humana subsidiary, seeks a Telephonic Nurse Care Manager working remotely. You will support discharged patients and those needing post-acute services, guiding care options and Medicare aspects by phone with empathy.

Requirements include an RN with compact-state licensure, 3+ years in post-acute nursing, and knowledge of home health. Travel to Humana offices may occur; 19 hours weekly, weekend coverage, and holidays may be required.

Qualifications

• Associates Degree in Nursing required; BSN preferred.
• Compact state RN license required.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Strong analytical and empathetic communication skills over the phone.

Responsibilities

• Be a CenterWell Home Health representative for patients discharged from service or needing post-acute services.
• Navigate options and educate on post-acute care, Medicare coverage, and resources.
• Determine clinical level of care from data and match to service sites; advocate for patients.
• Assist admissions as an ambassador for appropriate candidates and ensure timely referrals.
• Maintain performance goals and patient satisfaction, participate in trainings.

Education

Associate degree in nursing

RN license (compact state)
