Job summary

Humana Inc. is seeking a Telephonic Registered Nurse Care Manager to support patients transitioning into and out of Home Health, Hospice, and Palliative Care.

This work-from-home role reports to the Manager, Care Management and focuses on ensuring optimal continuity of care across post-acute services. Responsibilities include clinical care determinations, referral facilitation, patient education, and collaborating with the clinical team to improve patient outcomes.

Responsibilities

• Advise on post-acute care options and coordinate patient referrals.
• Determine appropriate level of care based on medical information and patient needs.
• Educate patients about Home Health, Hospice, and Palliative Care services.
• Support admissions by acting as a patient advocate throughout the referral process.
• Maintain compliance with HIPAA and company policies.
• Contribute to department performance metrics and patient satisfaction goals.
• Complete training and participate in special projects as assigned.
