Job summary

CenterWell Home Health is seeking a Telephonic Nurse Care Manager to support patients transitioning between services. You will work remotely, collaborating with the clinical team to facilitate follow-up and optimize post-acute care, including home health, hospice, and palliative care.

You will determine appropriate levels of care based on discussions and medical data, ensuring timely referrals, patient advocacy, and adherence to HIPAA and company policies.

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license; additional state licensures may be obtained.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Learn and master information related to locations and services of clients.
• Analytical and problem-solving abilities.
• Excellent verbal and interpersonal skills.
• Communication with empathy over the phone.

Responsibilities

• Be a CenterWell Home Health representative for discharged patients or those needing post-acute services.
• Provide patient options and education using multiple tools; address location lookup and inquiries.
• Make clinical level of care determinations based on data and refer to appropriate services.
• Support department performance goals and patient satisfaction targets.
• Assist admissions by guiding eligible patients through the process.
• Place the right patient in the right care setting at the right time.
• Complete mandatory trainings including HIPAA and ethics/compliance.
• Educate patients on Home Health, Hospice, and Palliative Care services.
• Review clinical eligibility for alternate services and participate in special projects.
