Job summary

Humana Inc. is seeking a Telephonic Nurse Care Manager to work remotely, coordinating care for patients transitioning between services. The role reports to the Manager, Care Management and focuses on post-acute care, home health, hospice, and palliative options.

Required are an Associates Degree with a compact RN license and at least 3 years of post-acute nursing experience. BSN is preferred, with strong communication and analytical skills to drive patient outcomes.

Qualifications

• Associates Degree required; BSN preferred.
• Compact RN license required; multiple state licensure may be needed.
• At least 3 years of post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.

Responsibilities

• Serve as a telephonic Care Manager to support patients transitioning into/out of services.
• Provide patient education on Home Health, Hospice, and Palliative Care options.
• Assist in admissions processes and referral follow-ups to optimize timely care.
