Job summary

Humana Inc. is seeking a Telephonic Registered Nurse Care Manager to support patients transitioning into and out of CenterWell Home Health services. You will work remotely, collaborating with the clinical team to ensure optimal post-acute care and timely referrals.

Requirements include an Associates Degree (BSN preferred), a compact state RN license, and at least 3 years in post-acute nursing. Strong verbal, interpersonal, and empathetic communication skills are essential.

Qualifications

• Associates Degree required. BSN preferred.
• Compact state RN license required; additional state licenses may be obtained.
• At least 3 years post-acute nursing experience; home health or hospice experience preferred.
• Knowledge of home health, hospice, and palliative care services.
• Excellent verbal and interpersonal skills; communication with empathy over the phone.

Responsibilities

• Be a CenterWell Home Health representative, supporting patients discharged from service and those needing post-acute services.
• Navigate healthcare options, care services post-acute offerings, Medicare coverage, billing issues, and accessing resources.
• Make clinical level of care determinations based on discussion, records, and data; match needs to service sites and advocate for timely referrals.
• Assist admissions by acting as an ambassador for patients meeting admission requirements; focus on placing the right patient to the right care setting.
