Job summary

Humana Inc. is seeking a Telephonic Nurse Care Manager to support patients transitioning to and from services. This remote role requires a compact RN license, 3+ years in post-acute nursing, and knowledge of home health, hospice, and palliative care.

You will collaborate with the CenterWell clinical team to optimize patient outcomes and adherence to care plans. Requires an Associate's degree (BSN preferred), strong communication skills, empathy over the phone, and ability to navigate post-acute

Qualifications

Associates Degree required; BSN preferred.
Compacts state RN license; additional licensure may be needed.
Minimum 3 years post-acute nursing experience; home health or hospice preferred.
Knowledge of home health, hospice and palliative care services.
Ability to learn locations and services of clients.
Analytical and problem-solving skills.
Excellent verbal and interpersonal communication; empathy over the phone.

Responsibilities

Support patients transitioning from service; navigate post-acute options and Medicare coverage.
Provide education on home health, hospice, and post-acute care services.
Assist admissions and ensure timely referrals and appropriate care settings.
Communicate with clinical team to follow-up on referrals and outcomes.
Adhere to HIPAA, ethics, and company policies; participate in training.
