Job summary

Humana Inc. is seeking a Telephonic Nurse Care Manager to support patients transitioning into and out of CenterWell Home Health services. This remote role reports to the Manager, Care Management and requires knowledge of post-acute care, hospice and palliative services.

You will communicate with the clinical team, educate patients, and facilitate timely referrals while upholding HIPAA and compliance standards. Training may occur outside standard hours.

Qualifications

• Associates Degree required. BSN preferred.
• Compact state RN license may be required.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Learn and master information related to locations and services of clients.
• Analytical and able to problem-solve.
• Excellent verbal and interpersonal skills.
• Empathetic communication over the phone.

Responsibilities

• Be a CenterWell Home Health representative assisting patients discharged from service or needing post-acute services.
• Use various tools to provide patient options and education on post-acute care and Medicare coverage.
• Determine clinical level of care based on discussion and records; match to service site or propose alternatives.
• Follow up and track referral and admission outcomes to maximize admissions.
