A leading healthcare provider in Singapore seeks a qualified pharmacist to deliver pharmaceutical care in inpatient and outpatient settings. The role includes running ambulatory clinics and ensuring appropriate drug therapy for patients. Candidates should have a relevant degree and a minimum of three years of clinical experience, with excellent communication skills preferred.
You will provide good pharmaceutical care to our patients in inpatient and outpatient settings. You will have opportunities to run ambulatory clinics such as the Anticoagulation Clinic, Cardiology, Geriatric, Renal and Smoking Cessation Pharmacist Clinics. You may provide services such as the Antimicrobial Stewardship Programme (ASP), Inpatient Anticoagulation Consults (IPAC), Integrated Diabetes Control Programme (IDCP), and take care of critically ill patients in the Intensive Care Units (ICU). On a daily basis, you will review patients’ prescriptions and medication charts for appropriateness of drugs prescribed and perform interventions on drug-related problems. Where applicable, you will recommend and monitor patient-specific drug therapy. You will dispense medications to patients, explaining the indications and side effects for drug therapy and instructions for administration. Where needed, you will provide drug information to other healthcare professionals.
Responsibilities include daily review of patients’ prescriptions and medication charts for appropriateness, performing interventions on drug-related problems, recommending and monitoring patient-specific drug therapy where applicable, dispensing medications with explanations of indications and side effects, and providing drug information to other healthcare professionals as needed.
