Overview
Position Overview: As a Social Worker, you will be working directly with youths and their families within the community. Your focus will be on providing meaningful interventions, strengthening reintegration efforts, and fostering positive youth development through collaborative case management and stakeholder engagement.
Responsibilities
- Individualized Case Management Conduct case orientation and develop tailored intervention plans from the outset. Use appropriate frameworks to set clear, achievable goals and review progress periodically.
- Multi-Stakeholder Collaboration Work closely with Probation Officers (POs), Child Protection Officers (CPOs), schools, families, and employers for joint case management and holistic care.
- Community & Partnership Development Build and maintain partnerships with schools, employers, social service agencies, and other community stakeholders to support the youth’s reintegration.
- Case Presentation & Progress Reporting Attend and present at various meetings and review platforms to provide updates on intervention progress.
- Documentation & Compliance Maintain accurate and timely case notes and documentation in accordance with organizational guidelines and standards.
- Operational Coordination Liaise with the Hostel Operations & Development Department to ensure accurate information is provided for daily scheduling and movement planning.
- Group Work Facilitation Conduct and participate in group-based interventions or activities for youth residents.
- Other Duties Support ad-hoc tasks and initiatives as assigned by supervisors or management.
Technical Competencies
- Solid understanding of relevant social work theories and assessment tools.
- Ability to design case plans that are both practical and outcome-focused.

Preferred Qualifications & Experience
- Bachelor’s degree in Social Work or a related field.
- At least 2 years of relevant experience in the social service sector or a similar youth-focused environment.
- Familiarity with clinical social work practices and a desire to further develop therapeutic skills.
- Enthusiastic about working with at-risk youth and contributing positively to their growth and reintegration.
