Job Description:
- Responsible for supporting Medical Social Workers (MSW) team in direct patient work, administrative work and general duties
- Manage routine cases assigned by MSWs independently
- Provide training for new social work and administrative assistants
- Conduct financial assessment on eligibility of applicants for the various financial schemes
- Conduct field visits and facilitate discharges by accompanying patients home or to social service organizations
- Able to independently take on inpatient and outpatient casework, doing follow up work for discharged patients and networking with community services to facilitate seamless care
Job Requirements:
- Diploma with minimum 1 year of experience in social service agency
- Certificate in basic counselling skills or related skills will be added advantage
