Provide personalized patient care management and ensure patients submit all required program documentation in a timely manner.
Conduct scheduled outbound calls to enrolled patients and handle inbound calls, including FAQ-related inquiries; accurately record and maintain all patient interaction details in the PatientCare Management System.
Support the delivery of programs in accordance with defined service levels and standards, including case handling time, established protocols, patient data confidentiality, and adherence to program Standard Operating Procedures (SOPs).
Follow best-practice medical guidelines and guidance from the Operations Manager and medical advisory team; responsibly report any Adverse Events identified during patient interactions.
Prepare periodic program reports and assist with audit preparation and project checks; participate as an auditee in both internal and external audits when required.
Perform any other duties as assigned by the Operations Manager or Program Manager.
NITEC or Diploma qualification in any field.
Relevant healthcare or customer service industrial attachment / working experience.
Proficient computer skills for data entry and reporting.
Ability to obtain, record, and maintain accurate information (e.g. patient notes, statistics).
Ability to evaluate problems and make timely decisions.
Knowledge of SAP is preferred.
Proficient in Microsoft office and related software.
