A community healthcare organization is looking for a Registered Nurse based in Singapore. The role involves providing clinical assessments, mentorship, and training for staff within the community care and hospice services. Ideal candidates should possess a Bachelor's Degree in Nursing and relevant clinical experience. Responsibilities include developing care plans, supervising nursing staff, and leading quality improvement initiatives. This position requires a commitment to enhancing nursing practice and patient care.
Provide independent clinical assessment of patients and formulate individualized care plans within the scope of practice of a Registered Nurse and in consultation with the multi-disciplinary team, referring appropriately for internal or external services as necessary
Provide guidance to junior nurses on management of patients with complex care needs
Provide after-hours phone call advice to patients and their caregivers on a scheduled shift-work basis
Act as an expert resource for nurses and mentor colleagues to aid them in the advancement of nursing practice in providing home-based health care and palliative care in MWS services.
Develop and provide orientation / induction programmes and preceptorship for new staff
Develop and coordinate regular training for nurses and ensuring regular competency measurement for nurses
Supervise assistant nurse clinicians/managers and junior nurses in carrying out their responsibilities
Ensuring adequate coverage for daily operational needs
Lead and implement strategies and initiatives to promote well-being of nurses
Develop internal clinical audit frameworks and conduct audits to ensure compliance with nursing standards
Lead quality improvement initiatives
Assist HOC/Senior Nurse Manager in reviewing and implementing relevant nursing policies and procedures
Any other tasks that may arise as part of MWS service requirements.
Registered Nurse with a valid practicing certificate from the Singapore Nursing Board.
Bachelor’s Degree in Nursing is required.
Advanced / Specialist Diploma in Gerontology, Palliative Care, Community Nursing or related fields
Minimum 8-10 years of clinical experience, with at least 3-5 years in community or hospice care.
Clinical experience preferably with exposure to ICU, Palliative or Geriatric care.
Minimum 3-5 years in providing clinical training to nurse
