Job description

Responsibilities

• Collecting Medication History: Gathering a comprehensive and accurate list of all medications a patient is taking (prescription, over-the-counter, supplements) from various sources, including the patient/carer, GP offices, and community pharmacies.
• Comparing Medication Lists: Comparing the patient's current home medication list with the new medication orders prescribed by the physician to identify any discrepancies.
• Resolving Discrepancies: Investigating and resolving identified discrepancies by consulting with the prescribing physician and other healthcare professionals to ensure the most clinically appropriate plan is in place.
• Documenting and Communicating: Documenting the reconciled medication list and all changes in the patient's record, and communicating the updated plan to the patient, caregivers, and other involved healthcare providers.
• Providing Patient Education: Counselling patients on their medications, including new medications, dosage changes, and potential side effects, to ensure understanding and adherence.

Job Requirements

• Min Bachelor of Science in Pharmacy from an accredited college by Singapore Pharmacy Council.
• Registered with the Singapore Pharmacy Council.
• Able to work independently in a fast-paced environment with accuracy and in a team.
