MINDS is today one of the largest charities in Singapore employing more than 850 staff. With the key focus to expand community-based services, improving the employability of PWIDs, strengthening education, and engendering inclusive community living, MINDS offers a holistic range of services and programmes across the PWIDs' lifespan.

RESPONSIBILITIES

Life Coaches provides training, intervention and assistance to Persons with Intellectual Disabilities (PwIDs) to enhance their community living and independent living skills. They also work closely with caregivers and PwIDS in discussing future care plans and building up their support network with the support of social workers and community partners.

Community-Based Support Services (CSS) is the dynamic and nimble service delivery arm in MINDS that responds to emerging needs of PWIDs and Caregivers in the community. Other services include Me Too! Club, Home Based Care Services, Caregivers Support Services, Special Student Care Centres and Disability Case Management Programme, MINDS Clinic to enhance community inclusion and participation of caregivers and PWIDs in the community.

As part of the Future Care Planning Team, the Life Coaches play a pivotal role in the training and well-being of PwIDs towards independence. Using a person‑centred approach, he/she works towards improving their independent living and confidence to access the community with the final goal of enhancing the client's quality of life.

He/she maintains up-to‑date documentation of clients' conditions and learning progress. He/She regularly provides inputs for the development and review of future care plans. A caring, respectful, and patient professional, the Life Coach works with various community stakeholders and partners to advocate for the needs of clients to live with support in the community.

Provide Client Training and Life Coaching Skills
  • Provide training, support, and activities that help clients manage their community living needs and achieve goals set in care plans.
  • Guide and provide clients with information on social support services available at other organisations.
  • Respond to challenging behaviours of clients.
  • Recommend measures to mitigate risk to clients.
Develop training materials Administration of Future Care Plans and ensure proper documentation
  • Development and review of training and care plans.
  • Maintain accurate and up‑to‑date documentation of clients' progress and incident reports.
  • Follow advisories concerning basic health, hygiene, and safety to provide a safe and healthy physical environment.
  • Apply technology effectively for work and teaching.
Conduct programmes and activities for clients
  • Contribute to the design and conduct of social, recreational and educational activities to promote independence, health, wellness, and quality of life.
  • Guide clients in the development of basic personal and social skills, independent living and communication skills.
  • Identify areas of improvement in programmes, activities and training.
QUALIFICATIONS
  • Minimum Diploma.
  • Disability Studies and special education will be an advantage.
OTHER INFORMATION
  • Added advantage for those with experience in education/training of persons with intellectual disability or multi‑disciplinary team environment.
  • Self‑motivated and fully able to participate in on‑site and off‑site activities.
  • Basic First Aid will be an advantage.
