Key Responsibilities

Clinical & Nursing Procedures

• Provide post-surgical management including:
• Administration of injections (IV, SC, IM)
• Care of central venous lines
• Tracheostomy care
• Management of drainage tubes (e.g., surgical drains, chest drains)
• Perform wound management and provide appropriate wound care.
• Maintain and change female urinary catheters and drainage systems as applicable.
• Provide stoma care (colostomy / ileostomy), including bag changes and peristomal skin care.
• Monitor and support pain control in collaboration with managing physician.
• Perform nasogastric tube (NGT) insertion and tube feeding.
• Assist in bowel management (e.g., enema, manual evacuation) based on care plan.

Ensure safe medication management including

• Administering medications
• Medication packing and supervision
• Monitoring compliance and side effects
• Complete accurate and timely clinical documentation and reporting.

Caregiver Support & Education

Provide caregiver education and training in:

• Prevention of falls and pressure sores
• Proper feeding and transfer techniques
• Provide lifestyle guidance including nutrition counseling, ADLs advice, and safety planning at home.

Requirements

• Full Registration with SNB (Singapore Nursing Board) and valid BCLS Certification.
• Advanced Diploma in Gerontology or Palliative Care is an advantage.
• Preferably with experience in: Palliative care / continuity care/ Acute Care
• Strong interpersonal and communication skills; meticulous in patient care and documentation.
• Able to commit to 12-hour shifts (day & night occasionally )
• Holding a valid Singapore driving licence will be an advantage
