Job summary

A nursing facility in Singapore is seeking a dedicated Nurse to provide competent care to residents. Responsibilities include assessing resident needs, coordinating care plans, and maintaining documentation. Job description
Responsibilities

1.1 Work under supervision of SNM / NM / DNM / Supervisor / RN

1.2 To analyse, plan, implement and review nursing care for residents. Co-ordinate and implement care plan for the residents.

1.3 Assess Resident’s needs on admission and each shift throughout their stay in Nursing Home.

1.4 Conduct rounds to ensure residents are well and safe and report any abnormality for immediate / timely treatment and follow up on progress of residents with SNM / NM / DNM / Supervisor / RNIC / RN.

1.5 Prepare and assist Physician with procedures.

1.6 Assist in identifying problems related to illness and decline in functional and cognitive abilities, and advocate early medical attention.

1.7 Assist in assessing residents after incidents occurred, recommend preventive measures and complete Incident Report 1.8 Assist in reviewing the RAF of residents in the branch.

1.8 Maintain and ensure accurate documentation of all care rendered as required by the Company’s quality procedures and guidelines

1.9 Communicate and discuss resident’s response to treatment / interventions / activities and progress with Physician, nursing colleagues and allied health care team assign to Resident.

1.10 Deliver safe and competent nursing care

1.11 Provide Resident / Family on information and education on delivery of care, process, disease prevention, treatments, and interventions in timely, accurate and concise manner.
