JOB PURPOSE
The jobholder is responsible to provide personalised, and cost-effective care to the patients in the nursing home in accordance to established policies and procedures, standards of nursing care and practices.
Duties and Responsibilities
Primary Responsibilities and Duties (80%)
- 2.1.1 Complies with approved standards of nursing practices.
- 2.1.2 Assists the RN to supervise care staff and healthcare attendants.
- 2.1.3 Provides basic nursing care to the patients.
- 2.1.4 Updates on changes in patient’s conditions and needs.
- 2.1.5 Ensures care delivery and nursing interventions as outlined in the care plan.
- 2.1.6 Carries out nursing rounds with the RN.
- 2.1.7 Supervises care staff to ensure comfort and good personal hygiene of patients under his/her care.
- 2.1.8 Supervises care staff to check, monitor and record patient’s vital signs.
- 2.1.9 Maintains accurate documentation of care given.
- 2.1.10 Assists RN in serving medications to patients and comply with safe medication practices.
- 2.1.11 Assists and guides care staff in the transfer and positioning of non-ambulant patients.
- 2.1.12 Supervises care staff in diet serving and feeding of patients that require help.
- 2.1.13 Supervises care staff in ensuring comfort and good personal hygiene of patients under his/her care.
- 2.1.14 Assists in the admission and orientation of new patients.
- 2.1.15 Performs rehabilitation activities such as motion exercise and walking with patients.
- 2.1.16 Organizes and sends patient for appropriate therapy as prescribed, example occupational therapy.
- 2.1.17 Co-ordinates visits for medical check-ups and transport arrangement.
- 2.1.2 Accompanies patient to the hospital/polyclinic for follow-up appointments.
- 2.1.3 Initiates actions in emergency situations; escalates and provides supports to RN promptly.
- 2.1.4 Works with RN to ensure a safe and clean environment in the home.
