Roles and Responsibilities
  • Home Care services for patients transiting from hospital to home
  • Any other community related duties will be assigned.
  • Help with discharge planning and post-discharge care coordination.
  • Conduct and document nursing assessments.
  • Develop individual care plans based on patient needs.
  • Provide necessary nursing services (e.g., feeding tube, catheter, dressing changes).
  • Assist with administrative tasks as directed by senior staff.
  • Perform any other nursing duties when required
  • Facilitate and participate in Multidisciplinary Team meetings to provide smooth delivery of patient care and services.
Job Requirements
  • Diploma / Degree in Nursing
  • Registered with Singapore Nursing Board
  • Possess a valid BCLS or CPR/AED certification.
  • At least 5 years of relevant working experience in the related field
  • Post-registration experience in a hospital or hospice setting preferred
  • Oncology / Palliative Care /Geriatrics / Gerontology knowledge is an added advantage
  • Procedure skill set: NGT insertion and change/ Catheterization/ Phlebotomist/ Stoma bag change/ Tracheostomy care/ Suctioning
  • Demonstrate competency in clinical nursing skills and knowledge such as performing IV cannulation, venipuncture for blood sampling and IV administration of medical fluids.
  • Available to perform rotating shifts including weekends and public holidays when required
  • Strong organizational, coordination, interpersonal and communication skills
  • Able to converse in local languages is preferred
  • Compassionate, patient, and dedicated to providing high-quality care
  • Good interpersonal and communication abilities, in written and spoken English
  • Proficient in computer skills and Microsoft office
Work Location

Locations include Singapore and international opportunities as part of the program.

Shift Hours

1) 8am to 8pm (Day Shift)

2) 8pm to 8am (Night Shift)

Able to work on weekdays and weekends according to roster

Locums able to commit 2 shifts per week are welcome to apply
