Duties & Responsibilities
- Conduct comprehensive biopsychosocial assessments to understand residents’ social support, mental health needs, coping abilities, and environmental factors.
- Develop individualized care plans in collaboration with residents, families, and multidisciplinary teams, with measurable goals and tailored interventions.
- Provide supportive counselling to residents and their families, addressing emotional needs, concerns, grief, and adjustment issues.
- Identify and coordinate internal and external resources to meet residents’ psychosocial, emotional, and practical needs.
- Liaise with community partners, SSAs, hospitals, and government agencies to ensure appropriate support.
- Participate actively in discharge planning meetings, assisting residents and families with transitions to community living, step-down care, or long-term arrangements.
- Maintain accurate and timely documentation of assessments, care plans, progress notes, and case updates in compliance with regulatory and audit standards.
- Provide guidance to families on coping strategies, available resources, advance care planning, and end-of-life considerations.
- Maintain up-to-date knowledge of geriatric care, counselling practices, social service frameworks, and case management standards.
- Support Home-wide programmes, initiatives, and community engagements.
- Perform any other duties as assigned by the Social Work Supervisor or Head of Home.
Requirements
- Bachelor’s Degree in Psychology, Counselling, or related field.
- Relevant experience in healthcare, eldercare, or geriatric social service settings preferred.
- Proficient in psychosocial assessment tools, counselling techniques, and case management approaches.
- Strong interpersonal skills with the ability to build rapport with elderly residents and their families.
- Skilled in crisis intervention, problem-solving, and advocacy.
- Familiarity with Singapore’s social service ecosystem, community resources, and support networks.
- Commitment to ethical practice, confidentiality, cultural sensitivity, and diversity.
