Job description

Wecurrently have a number of exciting opportunities for the appointment ofdynamic, self-motivated Band 6 Sisters/Charge Nurses to work as part of thePatient Flow Coordinator Team within Acute Medicine based at St JamessUniversity Hospital Leeds.

Weare looking for enthusiastic individuals to take responsibility and activelymanage patient flows across the Clinical Service Unit by implementing andmaintaining processes that safely expedite patient placement and discharge.

Thejobholder is responsible for the timely and appropriate placement of patientsrequiring admission to the Trusts bed base and for the escalation procedurefrom the Emergency Department.

Therole will give you an insight into the wider organization and the issues anddrivers which shape current NHS services. You will develop advanced skills inidentifying risks and solving complex issues, and gain valuable experience inbalancing staff availability and skills against the needs of patients acrossthe service.

Main duties of the job

Youwill be required:

Toassist in the delivery of care standards and quality indicators to supportsafe and effective patient care.

Youshould have knowledge in the field of hospital discharge and an understandingof the requirements of the national hospital discharge and community supportpolicy and operating model.

Toact as a positive role model.

Thesuccessful applicant will need to demonstrate recent change managementimplementation, and a clear understanding of Leeds Teaching Hospitals Nursing& Midwifery Strategy.

Job responsibilities

JOBPURPOSE

Thejobholder will play an essential role assisting the Senior Sister/ChargeNurse with the 24 hour continuing responsibility for the effective andefficient management of the ward / department. They will contribute to the provision ofprofessional, clinical, managerial leadership and supervision for the nursingteam. The jobholder is responsible forhelping to develop and maintain a ward or departments philosophy that has thepatient at the centre of compassionate safe and quality care delivery. By assisting with managing theadministrative and financial aspects of the ward, the jobholder will supportthe cost effective use of resources.

Asan experienced nurse the jobholder will assist in co-ordinating activities onthe ward or department ensuring effective communication is maintained at alltimes, providing support to qualified and unqualified nurses within theward/dept. He/she will play a key rolein the assessment of patient care needs and the development, implementationand evaluation of programmes of care.

Thejobholder will act as a role model, promoting quality evidence-basedpractice, developing the nursing team and ensuring effective management ofthe ward/department in the absence of the Senior Sister/Charge Nurse.

JOBDIMENSIONS

Workefficiently to facilitate the quick movement of patients from the emergencydepartment into the medical bed base. The jobholder will need to be competentin multi task management and be able to make evidenced based decisions. Thejobholder needs to work well in high pressured situations and be confident tocommunicate with all levels of the multidisciplinary team.

Thejobholder will communicate the Clinical Service Units position to the seniorteams and escalate any issues that need resolving.

Qualifications

Registered Nurse (RN Adult foradult clinical areas or RN Child for childrens areas (Level 1 or 2)

Recognised teaching/assessingqualification (desirable)

Experience

Significant post registrationexperience at staff nurse level or equivalent

Experience of clinical audit andstandard setting

Working with a wide range ofprofessionals including medical, nursing and management colleagues

Clinical experience within thespeciality (desirable)

Supervise ward staff with regardsclinical practice and patient experience to promote the delivery of excellentclinical standards

Provide a visible clinical lead androle model to the nursing and widerteam by delivering high quality compassionate care as an expertpractitioner

Ensure that documentation standardsare maintained in accordance with NMC and LTHT guidelines, whether written orcomputerised.

Promote and maintain positiveworking relationships between all staff, internal and external to theTrust, involved in the care ofpatients

Help to set, monitor and seek tocontinuously improve standards and the quality of patient care, includingthose standards defined in local and national guidance
