The post holder will be employed by North West Leicestershire Federation and this position would be based at Whitwick Health Centre.

Main duties of the job

You will be responsible for assessment and specialist employment advice, support and interventions. You will manage a caseload of people assessing individual barriers into employment, experienced through physical health, mental health and social situation. Using the WorkWell pathway work plan, you will prescribe either multi-disciplinary in-house support or triage/signpost, refer to and collaborate with the care navigation team.

You will facilitate clients being referred to the service quickly so that they do not remain unsupported whilst on sick leave and support them to return to work swiftly (where appropriate), or to follow the destination that is best suited to their circumstances, which enables individuals to achieve positive service experience outcomes.
