Overview

Urgent Response is a community based service. We are looking to recruit 2 motivated, dynamic and flexible Band 6 Senior Nurses within this service. This is a secondment / fixed term post for 12 months. We provide unplanned and urgent care as part of a multi-disciplinary community based team. The main base is in Nuneaton; the work covers North Warwickshire because the patients will have a Warwickshire GP. The role involves working as a member of the Urgent Response / Community Response team and responding to visits via Clinical Triage.

Responsibilities
  • Lead as a member of the Urgent Response Team and respond to urgent and unplanned visits originating via Clinical Triage.
  • Assess, plan, coordinate, deliver and evaluate programmes of care provided across health, social and voluntary care sectors as part of a multi-disciplinary community based rehabilitation team.
  • Work in the community, in clients’ homes, residential and nursing homes and other sites as required by the service.
  • Maintain a holistic, patient-centred approach as part of a multi-disciplinary team.
Qualifications and Requirements
  • Minimum requirement: experienced Band 5 nurse who wants to progress to a Band 6 role and receive development support to assist in achieving this aspiration.
  • Experience in leadership within urgent response, and in assessing, planning, coordinating, delivering and evaluating care across health, social and voluntary sectors as part of a multi-disciplinary community-based rehabilitation team.
  • Willingness to work within the community, clients’ homes, residential homes and other sites as required by the service.
