A healthcare organization in Shefford is seeking an individual to work within a multidisciplinary team, providing therapeutic interventions in diverse settings such as clients' homes and clinics. Responsibilities include assessing and implementing personalized care plans, managing caseloads, and ensuring effective communication with clients and their families. Candidates should have a strong knowledge of community support services and be able to work autonomously, fulfilling various shifts including weekends. This role aims to improve clients' quality of life through comprehensive care.
Responsible for assessing, planning and implementing therapeutic interventions. To assess, demonstrate and provide appropriate, adaptive equipment to meet specific client needs and to facilitate safely in the chosen environment.
To form professional relationships with clients, family and carers and communicate condition related information to them in a way that respects their views, autonomy and culture.
To involve clients, family and carers in the planning of care/rehabilitation programs and encourage self-management where appropriate. Promoting self-management of health related conditions.
To have an enhanced level of knowledge of support services within the community.
To communicate effectively with all members of the Integrated Community Services team. To support and promote wider team working.
To provide written reports and verbal feedback to the team regarding clients’ performance/progress in areas of self-maintenance, productivity and leisure.
To act as named person for the client to contact within the department and to feedback relevant issues to senior staff.
Maintain professional, responsible and appropriate relationships with managers, colleagues, clients, their family/friends and other services.
To support other departments and localities within the Therapy Services at the request of management as demand requires.

To provide advice, teaching and instruction to relatives, carers and other professionals to promote the understanding of the aims of therapy and to ensure a constant approach to patient care.
To undertake advanced holistic therapeutic assessments with a diverse range and complexity of physical and psychological conditions which increase the risk of falls.
To hold own caseload of clients referred. Taking responsibility for accepting and discharging these patients, undertaking all aspects of clinical duties required within the pathway as an autonomous practitioner.
To ensure the client record on Systm1 contains the client’s full journey in a timely manner.
