Overview

The Community Falls and Fracture Liaison Service is looking for an enthusiastic, motivated person to join our team alongside our experienced Physiotherapy and Nursing staff.

Responsibilities
  • assist our team in the delivery of treatment of patients, working in their own homes or in residential settings.
  • take an active role in coordinating, delivering and evaluating individual patient care.
  • travel to different geographical locations across the Trust footprint in line with service needs; a full UK driving licence and access to own vehicle would be desirable for this community role.
