Job description

Following the direction and supervision of the Clinical Lead / Health Care Professionals, you will carry out assigned tasks and duties to assist with the physical, emotional and social care of patients. The service uses remote monitoring technology for vital sign monitoring and care plans, including home visits as needed, and works closely with urgent care response teams, district nurses, physiotherapists, occupational therapists and support workers.

Work in a sensitive, enabling manner and always be mindful of the diversity of the borough of Newham. The ultimate goal of the service is to avoid hospital admission where possible.

Monitor patient progress as defined by treatment aims, identify trends indicating deterioration and alert the community matron or GP. Provide shift coverage, including weekends when necessary.

Responsibilities
  • Work effectively as a member of the team, assisting in planning and implementing care plans and treatment programmes under the guidance of the Clinical Lead/HCP.
  • Remote monitor vital signs for patients on Telehealth and escalated to clinicians as required.
  • Work in the community and visit patients requiring insulin administration, eye drops, etc., according to the care plan.
  • Form professional relationships with clients and communicate in a way that respects their views, autonomy and culture, providing an integrated care approach.
  • Liaise with relatives, carers, medical staff, allied health staff and the multidisciplinary team to obtain or give feedback on patient care.
  • Contribute towards the team’s vision and attend meetings and events to promote the service.
  • Understand and apply confidentiality in all patient interactions.
  • Be legally accountable for all aspects of own work.
  • Use clinical knowledge and life skills to build rapport with clients, engaging and motivating them in their own care.
  • Maintain legible, accurate, contemporaneous clinical records.
  • Employ non‑specific skills to meet the psychological needs of service users with mental health needs through care planning and risk management strategies.
