A healthcare provider in London is seeking a dedicated individual to assist with the physical and emotional care of patients. The role involves monitoring patient progress, implementing care plans under supervision, and maintaining accurate clinical records. Following the direction and supervision of the Clinical Lead / Health Care Professionals, you will carry out assigned tasks and duties to assist with the physical, emotional and social care of patients. The service uses remote monitoring technology for vital sign monitoring and care plans, including home visits as needed, and works closely with urgent care response teams, district nurses, physiotherapists, occupational therapists and support workers.
Work in a sensitive, enabling manner and always be mindful of the diversity of the borough of Newham. The ultimate goal of the service is to avoid hospital admission where possible.
Monitor patient progress as defined by treatment aims, identify trends indicating deterioration and alert the community matron or GP. Provide shift coverage, including weekends when necessary.
