Overview

Job responsibilities

Your focus as a Team Leader will be to support the home management team in delivering high-quality, person-centred care. This will involve leading in the care planning process, assisting with discharges, monitoring resident wellbeing, and ensuring safe systems of work. You will need to demonstrate strong leadership, communication, and decision-making skills to succeed in this role.

Other responsibilities include:

  • Leading in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations; completing care plan audits; developing action plans; and being responsible for completing the action plans.
  • Assisting in the smooth transition of discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed where appropriate.
  • Monitoring the wellbeing of each resident, being aware of changes impacting care and care planning, and ensuring changing needs are met with updated written records.
  • Practising safe systems of work across a range of tasks and moving and handling of residents, assessing risks, and ensuring safety of residents, visitors, and staff.
