Overview

This role is open to all internal staff including those registered on our bank.

We are seeking a dynamic and committed Support Worker to join the Children\'s Hospital at home team (CHAHT) covering 7 days a week including bank holidays. CHAHT is an acute service to facilitate early discharge and prevent hospital admissions for children and young people so they can be cared for within their own home environment for administration of IV/IM antibiotics and review of acute episodes of illness.

The team is based at Darent Valley Hospital and works closely with the Paediatric Assessment Unit (PAU) and Paediatric Inpatient team to ensure appropriate referrals are received and facilitate the smooth transition of care from the hospital to the community.

Responsibilities
  • Support the nursing and medical teams in caring for children.
  • Participate in the delivery of care to enable early discharge and home-based treatment where appropriate.
  • Work flexibly to cover shifts on weekdays, weekends and Bank Holidays.
  • Support the CHAHT pathway in collaboration with hospital and community teams.
  • Occasionally travel by car to meet and assist trained nurses at clients\' homes.
Qualifications & Requirements
  • A commitment to continuing your own personal professional development.
  • The ability to work shifts on weekdays, weekends and Bank Holidays.
  • Previous experience of working in a childcare or maternity setting is an advantage but not essential.
  • Have use of a car to occasionally travel to meet & assist trained nurses at clients\' houses.
