A healthcare provider in Tiverton seeks a Support Worker for a 12-month fixed term position. The role involves working within a multi-disciplinary team to support clients with mental health issues, aiding their recovery through personalized interventions. Candidates should have NVQ level 3 in Health and Social Care and experience in caring for individuals with mental health difficulties. The position offers a pro-rata salary of £24,937 to £26,598 for 26.25 hours per week.
We have an exciting Support Worker Opportunity for someone to join the North and Mid STEP team on a fixed term or secondment basis for 12 months working 26.25 hours per week.
Our Early Intervention Psychosis team provides assessment and treatment for people aged between 14 and 65 who are experiencing psychosis.
The successful candidate will be working as part of a specialist Multi Disciplinary Team in the community provide a range of personalised interventions to support recovery and wellbeing.
The role involves working independently across North and Mid Devon which is a large locality and will require meeting clients in their homes and other community settings. It will suit someone with experience of mental health and psychosis and also someone who is able to work independently. They will be motivated to support clients in terms of their social, emotional and occupational needs including outdoor activities, independent living skills and supporting independence. People with lived experience are valued in this role.
The post holder will be an integral part of the team and be expected to attend regular team meetings and supervision. They will participate in training opportunities and co-work with professionally trained staff to provided quality interventions as part of personalised care planning.

Under the daily supervision of mental healthpractitioners, the post holder will work as part of the multi-disciplinary teamto support the delivery of recovery care plans for clients with a range ofmental health presentations in the community. This will involve working acrossagencies e.g. Primary Care, Complex Care Teams and Third sectororganisations.
The post holder will work intensively with people tosupport them in their home, preventing hospital admission and facilitatinghospital discharge.
The post holder will be expected to work to a clearlydefined personal recovery plan outlining their role and the range ofinterventions required. This will include a significant monitoring role ofpeople on behalf of the team.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
