Your role and responsibilities

In this role, you'll build and maintain positive relationships with customers and their family carers, promoting health, wellbeing, and independence. You will support individuals through personalised care plans and short-term enablement strategies, offering guidance to help them make informed choices. At times, you will assist with dignified personal care needs and engage with customers and carers to explore alternative ways of meeting long-term needs. You will also promote services that enable safe, independent living at home, monitor progress against care plans, and maintain accurate records to support ongoing reviews.

This is a role where the rewards go beyond the day-to-day. You'll experience genuine job satisfaction and a strong sense of achievement, knowing your work directly helps people maintain their independence and continue living comfortably in their own homes.

This role involves travelling to people's homes in rural locations and driving in various weather conditions, including during hours of darkness, which may occur early in the morning or late in the evening. You will therefore require a valid driving licence and access to a car. If you believe you can meet the travel requirements through other means, please get in touch before submitting your application.
