A supportive community organization in the UK is seeking a Support Coordinator to assist individuals with complex needs in their journey towards independent living. This part-time role involves creating personalized support plans, providing compassionate care, and collaborating with a dedicated team. Ideal candidates are detail-focused with relevant experience and a passion for helping others. Salary is £13.21 per hour with additional benefits including paid leave, pension contributions, and training opportunities.

Join a supportive and compassionate team where your expertise is valued and your wellbeing matters. You'll help customers with complex mental health needs build skills for independent living. As part of our Central Resource Team you'll provide cover across services to ensure customers always receive the best support. Every day you'll make a real difference in helping them achieve their goals.
Create and develop support plans with customers and coordinate the support worker team.
Support individuals with complex needs and vulnerabilities. From homelessness, substance misuse, complex mental health and social isolation.
Assist with daily living activities; cooking, shopping, budgeting, tenancy upkeep and moving to independence.
Carrying out risk assessments, set goals and complete regular reviews and setting up tenancies for our accommodations.
Provide person-centred compassionate support without rushing.
Collaborate closely with colleagues for consistent quality support.

Passion for helping customers live their best lives with a collaborative and detail-focused approach.
Experience creating person-centred support plans and coordinating referrals.
Ability to work independently, stay calm under pressure and show resilience.
A vehicle insured for business use (mileage paid).
You'll join Laura on the Central Resource Team who has been with Home Group since 2022 and progressed from Support Coordinator through our training opportunities. She's supported by Meghan, our Client Service Manager for Yorkshire who also started as a Support Coordinator and is always available to help.
