Job description
  • To be responsible for an identified community caseload, ensuring high standard of professional conduct to comply with best practice and professional accountabilities.

Main duties of the job
  • To be responsible for an identified community caseload, ensuring high standard of professional conduct to comply with best practice and professional accountabilities.
  • To participate in the assessments and selection of potential clients ensuring that their physical/psychological and social needs are taken into account at all times.
  • Seek to establish a therapeutic relationship with individual clients and their families.
  • To ensure that clients in the community with behavioural problems, e.g. violent or potentially violent clients are identified in caseload and that caution and care is taken when visiting such clients, if necessary organising joint visits and being mindful of the lone worker policy.
  • To co-ordinate the day-to-day activity and manage the workload of the Team.
  • To ensure the legal requirements of the Mental Health Act (1983) and subsequent amendments are adhered to.
  • To establish and maintain good relationships with service users, carers and voluntary sector colleagues.
  • To provide best evidence practice to service users which complies with legislation, policies and procedures and relevant quality, governance and local and national guidelines.

Job responsibilities

To take a proactive approach to ensure all targets and standards set by the HCC, CSCI or Local Authorities relevant to the area of work/team are achieved.

  • To ensure systems are implemented for effective Health & Safety management.
  • All complaints will be responded to within the time specified, and incidents and near misses will be reported in line with Trust Policy.
  • Assist in ensuring results of research undertaken relating to national and international best practice are applied within the unit.
  • To develop close links with relatives/carers and ensure that effective communication takes place especially when clients are admitted or being prepared for discharge from hospital and that all appropriate agencies are informed, so that follow-up care can take place.

Person Specification
Education/ Qualification/ Training
  • Registered Professional in either Nursing (Mental Health) or Social Work, with current registration with NMC or HCPC.
  • Evidence of post registration continued professional development, qualifications or equivalent experience.
  • Educated to degree level or equivalent experience.
  • Post registration training or qualification in CBT, PSI or equivalent.
Experience
  • At least two years post qualification experience.
  • Previous experience of functioning as a team member.
  • Previous experience of Management and Clinical Supervision, including annual appraisal.
  • Previous experience of organisational change.
  • Previous experience of working within the NHS and with senior allied professionals, internal and external stake holders.
  • Experience of working in the community setting.
Knowledge and Skills
  • Good knowledge of current and relevant local and national agendas that impact upon Primary Care Mental Health teams and the wider health and social care system and the ability to understand and disseminate this knowledge to others.
  • Sound knowledge and understanding of the Mental Health Act (1983/ 2007/ 2014/5), Care Act (2014), relevant national standards and other relevant national policies.
  • Excellent knowledge & understanding of recovery principles and NICE guidance in relation to common mental disorders and the ability to ensure that evidence based practice with clear clinical outcomes is embedded into practice.
  • Demonstrate a competency in using electronic clinical systems and associated Microsoft Office products or equivalent software.
