A healthcare provider in the UK is looking for a Staff Nurse to provide clinical and professional leadership in the SMART team. The role involves triaging, assessing, and planning care for discharged patients, including administering IV treatments and conducting virtual visits. Successful candidates will need to communicate effectively, work both autonomously and within a team, and hold a nursing licence with relevant clinical experience. As a Staff Nurse you will provide clinical and professional leadership, supervision and support to patients and students within the SMART team. You will triage, assess, plan and implement the care of patients discharged home under the SMART service, attend SDEC/SAU/Hot clinic reviews, administer IV treatments, wound care (PICO and Vac dressings) and conduct virtual visits using our remote patient monitoring pathways. You will also provide care for OPAT (Outpatient Parenteral Antibiotic Therapy), heart‑failure and post‑operative patients, liaise with the multidisciplinary team and coordinate safe, effective and timely care practices.
