Overview

As a Staff Nurse you will provide clinical and professional leadership, supervision and support to patients and students within the SMART team. You will triage, assess, plan and implement the care of patients discharged home under the SMART service, attend SDEC/SAU/Hot clinic reviews, administer IV treatments, wound care (PICO and Vac dressings) and conduct virtual visits using our remote patient monitoring pathways. You will also provide care for OPAT (Outpatient Parenteral Antibiotic Therapy), heart‑failure and post‑operative patients, liaise with the multidisciplinary team and coordinate safe, effective and timely care practices.

Responsibilities
  • Lead and support patients and students within SMART team.
  • Triage, assess, plan and implement community care for discharged patients.
  • Attend SDEC/SAU/Hot clinic reviews and deliver in‑person care at patient homes.
  • Administer IV treatments, cannulation, venepuncture and wound care (PICO and Vac).
  • Conduct virtual visits using remote patient monitoring pathways.
  • Provide OPAT, heart‑failure and post‑operative care.
  • Liaise with multidisciplinary team to coordinate safe, effective, timely care.
  • Teach, audit, practice infection control and support research activities.
Qualifications & Skills
  • Competent in cannulation, venepuncture, general surgical wound care and drain care.
  • Ability to work autonomously and as part of a team.
  • Strong communication, decision‑making and confidence.
  • Full UK driving licence and own vehicle to visit patients in the community.
  • Registered nurse with appropriate clinical experience.
