Overview

We are recruiting for a Specialist Support Nurse - Renal Transplant who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.

The post holder will assess, plan, implement and evaluate care as a practitioner with specialist skills and provide advice and clinical expertise to other staff. They will carry a caseload and be responsible for the management of allocated cases, providing specialist nursing care programmes and support to patients and carers.

Responsibilities
  • Provide high quality, patient‑focused care by assessing, developing and implementing specialist nursing care programmes.
  • Offer specialist advice and support to and liaise with other staff.
  • Assess, plan, implement and evaluate care as a practitioner with specialist skills, and provide advice and clinical expertise to other members of staff.
  • Provide specialist clinical advice for patients and carers.
  • Carry a caseload and take responsibility for the management of allocated cases.
  • Strive to improve and maintain standards of care within the specialty.
  • Support other nurses by improving their skills to enable them to provide the highest standard of care for patients within the specialty.
