Overview
You will join a passionate and highly skilled interdisciplinary team committed to delivering long-term, person-centred therapy for individuals living with Parkinson's. Working closely with physiotherapists, occupational therapists, speech and language therapists, and Parkinson's specialist nurses, you'll play a key role in holistic assessment, treatment planning, and goal-focused rehabilitation. Your role will be primarily clinic based, but will incorporate participation in the rehabilitation exercise groups, and may involve elements of domiciliary working, depending on service need. You may also be involved in a cutting-edge research study investigating the use of AR technology to enhance therapy outcomes for people with Parkinson's, with opportunities for professional growth, research involvement, and skill development in digital health innovations.
As part of the role, you will participate in the approach, recruitment and follow up of patients into the research study under the direction of the clinical team, research nurses and research practitioners. You will provide clinical care to people with Parkinson's and support the research process as part of the multidisciplinary team.
Responsibilities
- To be professionally accountable and responsible for all aspects of own work.
- To adhere to and apply the Health and Care Professions Council's (HCPC) standards of proficiency and the Chartered Society of Physiotherapy Code of Members' Professional Values and Behaviour, and all other relevant guidance from the Partnership Trust.
- To establish and maintain effective working relationships within the Partnership Trust and with external organisations to meet service requirements and support the Trust's equality values.
- To assess new service user referrals and care/support needs. Assess service users and formulate, develop, implement and evaluate programmes of treatment/care/support for individuals, families and carers. The specialist care/support plans may include: promoting independence/self-management and maintaining safety; increased functional ability; improvement in health and wellbeing including mental health; interventions in disease/condition management; prevention and reducing health inequalities; admission avoidance; early supported discharge; case management/treatment/care plans; palliative and end of life care.
- To use professional knowledge and judgement to consider range of care/support packages and treatment options, taking into account service user's personal circumstances and preferences.
- To maintain and develop skills and knowledge to deliver a range of interventions and model good practice for other team members, including skill sharing.
- To maintain sensitivity to the emotional needs of service users and carers, offering reassurance and empathy.
- To work with individuals and carers who may find it difficult to engage with the service, including managing uncooperative or challenging behaviour.
- To use professional knowledge and expertise when working in changing environments and across professional boundaries.
- To maintain contemporaneous and accurate records as required by the Partnership Trust and in line with HCPC/CSP standards; support identified team members with guidance and direction where appropriate.
- To take responsibility for the safe use and transportation of equipment between locations; work in non-purpose-built rooms requiring movement of furniture and therapy equipment.
- To ensure that the skills of identified team members are deployed appropriately and that care is delivered by staff with the relevant education, expertise and competence.
- To deliver a high standard of intervention while managing unplanned demands using innovative problem-solving techniques.
- To provide advice, education and training to service users, carers and professionals regarding management and care/support of service users.
- To provide professional and clinical support and leadership to identified team members; support in identifying training needs and encouraging skill development in areas such as critical thinking, excellence in practice, and specialist knowledge and research-based practice.
- To take accountability for allocated assessments, planning and implementation of care packages/programmes of support within their caseload.
Qualifications
- Has proven experience at Band 5 or Band 6 level (neuro or elderly care experience desirable).
- Enthusiastic about innovation, research, and collaborative working.
- Demonstrates a strong commitment to evidence-based, person-centred care.
- Thrives in a multidisciplinary setting and is eager to develop specialist knowledge in Parkinson's.
- Is proactive, flexible, and passionate about delivering excellence in rehabilitation.
