Job description

An exciting opportunity exists for a Clinical Nurse Specialist in Palliative Care to gain experience working in a community setting (1.0 WTE). The post holder will work predominantly in the community setting with flexibility to work within the whole of the Specialist Palliative Care service as is required. This is a 7 day service which runs from 08:00 to 20:00.

The post holder will have substantial years post registration experience, to include palliative, cancer or non-cancer long‑term condition nursing. We will consider all transferable skills, but an ability to demonstrate a full holistic assessment and understanding of the principles of symptom control is key to this position.

We are looking for a highly motivated nurse with excellent communication skills to provide advice and support to patients, their families and health‑care professionals.

You will need to demonstrate an understanding of specialist palliative care and the context in which such care is delivered within the community setting.

Evidence of ongoing professional development is essential, and experience of teaching is desirable.

The role requires the postholder to provide specialist palliative care to patients with complex problems, act as a specialist resource to generic health and social‑care professionals and deliver education and training to develop their palliative care practice.

The role will require an ability to work both independently and as part of the multidisciplinary team, and to communicate effectively with a wide range of health‑care professionals.

  • Use advanced skills and expert knowledge to make comprehensive and focused assessments.
  • Use advanced skills and expert knowledge to identify subtle changes in condition and act on these appropriately.
  • Work in partnership with clinicians/health professionals to plan and implement treatment for palliative care and patients with life‑limiting illness.
  • Refer on to other services or professionals as appropriate. This may include facilitating discussion with the patient, their family and significant others concerning palliative care and end‑of‑life care plans.
