We are excited to offer an opportunity to join the Integrated Community Stroke Service, working as an Occupational Therapist / Physiotherapist delivering specialist stroke rehabilitation. The role involves completing comprehensive assessments, delivering evidence-based interventions and supporting patients to work towards individualised rehabilitation goals, primarily within their own homes or place of residence. You will play a key role in supporting timely and safe discharges from hospital, identifying and managing stroke-related complications early, and helping to reduce avoidable readmissions. The post also involves providing education and practical support to patients, families and carers, promoting self-management following stroke and offering advice and signposting to other appropriate services as needed. You will support patients within a defined geographical area, however, on occasion there may be a requirement to provide countywide cover in line with service needs.
