Job description

An exciting opportunity has arisen to work within the Children in Care Nursing team for one day a week. We are seeking a motivated, dynamic, committed practitioner who is enthusiastic about developing and providing a quality service to improve the health of Children in Care.

You will work as part of the Children in Care Nursing team to ensure that the health needs of Children in Care are identified and that services are provided in a coordinated manner, supporting children to meet their potential. You will be required to complete high quality review health assessments for children in care.

Experience of interagency, collaborative working and a clear understanding of the complex health needs of Children in Care are required.

Main duties of the job

• To undertaken health assessments and formulating health care plans for children and young people in care living in Derby City and within a 20 miles radius, signposting and making referrals to other agencies/services as appropriate.
• Act as an advocate for children and young people in care
• To work with social care colleagues to ensure the health needs of children and young people, identified at health assessments are addressed appropriately with on going follow up and review.
• To attend children in care reviews as appropriate with a particular emphasis on children and young people that are hard to reach.
• To contribute to the continuing development of an integrated, robust and modern service delivering high quality care to children, young people and their carers.
• To work closely with the health visiting and school nurse service to ensure the health needs of children in care are being met.
• To lead and implement health histories for young people leaving care
• Excellent written and oral communication skills.
• To be able to work independently and effectively within a small team.

Job responsibilities

The post holder will be required to identify and implement plans to meet the health needs for this very vulnerable group of children and young people. This will be achieved by improving the co-ordination, access and delivery of health care to children and young people in care both living in and out of area who are the responsibility of Derby City Local Authority.

• oQualified to diploma/degree level as Registered General Nurse, Registered Children's' Nurse or Registered Mental Health Nurse
• oExperience which includes working with children or young people with complex emotional needs.
• oPost registration qualification in teaching and assessing or evidence of experience and competency.
• oKnowledge and experience in contraception and sexual health advice.
• oKnowledge and experience in child and adolescent mental health
• oPost Registration qualification in Health Visiting / Community Practitioner Public Health Nursing or Specialist practitioner School Nursing
